Congregation Health Survey

15 questions · print or use online

Before you plan anything, ask. This survey takes three minutes to fill out and will save your team months of guessing. Copy it into a printed half-sheet, or into a free online form.

How to run it

  1. Keep it anonymous. People answer honestly only when no one can trace it back to them.
  2. Give it out in the service, with pens, and collect it before people leave. You will get five times the response of an emailed link.
  3. Offer the online version too for anyone who missed it. Free tools like Google Forms work fine.
  4. Never ask for weight, clothing size or a health history. It isn’t your business, and asking will cost you trust.
  5. Share the results with the church in broad strokes. People engage with something they helped shape.

Reading the results

  • Question 2 sets your topics. Teach what people asked for first.
  • Question 3 sets your guardrails. If cost is the top barrier, start with eating well on a budget.
  • Question 8 sets your calendar. Run what people said they would attend, and drop the rest.
  • Questions 11 and 12 are your volunteer list. Ask those people in person.
  • Question 15 goes to your prayer team, and stays private.

Keep completed surveys private, and shred the paper copies once you’ve tallied them.
